Coastal Primary Care network
Coastal Primary Care Network is an NHS collaboration between 4 GP surgeries. We provide quality NHS healthcare services to our patients and aim to support and connect with our local community.
Our surgeries are located in St Agnes, Perranporth, Carnon Downs and Chacewater. We all work together to give our patients a choice over where they visit, who they see and the services available. See below the surgeries that are part of our network:

What are primary care networks?
Primary care networks (PCNs) are groups of practices working together to build on the current primary care services whilst giving greater provision of proactive, personalised, coordinated and more integrated health and social care. Small enough to provide personal care whilst being large enough to benefit from the impact of collaboration. Cornwall and the Isles of Scilly have 59 GP practices, organised into 14 primary care networks.
The Primary Care Network has GP and practice manager representatives from all of the surgeries and we are committed to achieving:

As a PCN, we make sure patients have access to quality NHS services across our surgeries. We bring NHS services and local services together, such as community, mental health, social care and volunteering, to look after local patients and provide more integrated health and social care.
